About Chat 4 Heart Health

“Chat 4 Heart Health” is an NIH-funded study involving CU Anschutz Medical Campus, Clinic Chat, Denver Health, Salud Family Health Centers, and STRIDE Community Health Centers. The study aims to improve control of patients’ cardiovascular (CV) disease risk factors using cellphone-based text messages and a chatbot to improve control of American Heart Association’s Life’s Essential 8 (LE8) lifestyle factors: blood glucose, cholesterol, blood pressure, physical activity, weight, diet, sleep, and smoking.

You or someone you provide care for may have received a letter as you or they have been identified as a patient with a chronic cardiovascular condition, such as hypertension, diabetes, or hyperlipidemia, and are taking medications to treat the condition. We plan to send participants text messages to serve as resources on how to manage blood pressure, control cholesterol, reduce blood sugar, be active, eat and sleep better, lose weight, and stop smoking. In this study, we hope to follow patients over 2 years.

If you do not wish to participate, please complete the opt-out form and send it in the self-addressed, stamped envelope which were in your packet.
